  • By checking the box you understand: This program is intended to fall under the Tribe’s General Welfare Policy. It is the intent of the Tribal Council that the Program be funded from the Tribe’s general fund, and that Services provided by the Program will not be subject to state or federal taxation and will not reduce fixed income or other benefits of the recipients. However, recipients should consult with their own tax and/or legal professionals to determine actual tax and benefit consequences. To ensure that the Back to School PPE Benefit is used for Eligible Expenses, the Tribe reserves the right to request a receipt or other proof of payment s for any transaction from a Tribal Member at any time during the benefit cycle. The recipient understands repayment may be required from the recipient if the Tribe deems the funds to have been misused. The Tribe may pursue any available remedies at law and/or equity to recover misused funds. It is the responsibility of the Applicant to maintain proper records of invoices, receipts, and payments made toward eligible PPE expenses. I certify the information contained in this application is complete and accurate to the best of my knowledge. I understand that I am signing this application under penalty of prosecution if I knowingly give false information which results in payment to which I am not entitled. By signing I agree that the Education Department may contact other tribal programs for pertinent information as it applies to this application.

Purpose

The PPE Benefit was developed to help families with the costs associated with returning to in-person school and
meeting new guidelines for health and safety as it pertains to PPE. This benefit is intended for Snoqualmie Tribal
Members attending school, eligible Snoqualmie Tribal Minors, and eligible Snoqualmie descendants currently enrolled in
school.

Eligibility

All applicants must be enrolled members of the Snoqualmie Tribe, a biological minor child(ren) of an enrolled Snoqualmie Tribal Member, or an eligible Snoqualmie descendent.

All eligible applicants should be returning to an in-person learning classroom or facility, must have proof of enrollment in school on file, including the school’s name, student’s name and grade level. The following grade level categories are eligible.

  • Early Education aged children must be enrolled in a early education/ preschool program.
  • K-12th grade students must be enrolled in school full-time.
  • Higher Education or (AVT) students must be enrolled in a degree program, taking a minimum of one class toward their degree program or certificate program, and be earning a minimum 2.0 GPA.
  • Scholarship Higher Education or (AVT)- students must be enrolled in a degree program, taking a minimum of one class toward their degree program or certificate program, and be earning a minimum 2.0 GPA.

Benefit

Maximum Benefit Available per Eligible student: $100.00. All funding must be used towards the purchase PPE for the designated student for the purposes of in-person learning. PPE includes but is not limited to: face masks/shields, hand sanitizer, sanitization wipes, Kleenex, gloves, other PPE items as recommended by CDC for return to school.

Application Deadline

The purposes of this program is to provide health and safety items to students who are returning to in-person learning. To qualify for a check; a complete application with supporting documents must be emailed or postmarked no later than June 30th, 2021.
