Individuals who wish to receive health alerts must voluntarily opt in by completing the information below. By opting in, the individual agrees to receive health-related alerts, public health notifications, emergency information, and other important health communications from the Snoqualmie Indian Tribe’s Public Health Program.

Consent may be withdrawn at any time by contacting the Public Health program.
